    Towards the end of last year I wrote a letter to the Times newspaper. It was prompted by what had been a particularly gruelling night shift in casualty, where I had seen few cases that were either an accident or an emergency. I decided to unleash all of my accumulated anger, frustration, and fatigue in the form of a reply to a letter from a general practitioner suggesting that we should charge for the use of medical services. I furiously typed my response by email, and went to sleep at 10 am feeling much relieved.
